A four-month-old baby girl has died in Kuliyapitiya after being administered with the pentavalent vaccine.
The infant had been administered with the vaccine on 15 July at the MOH office and had developed complications later in the night.
Parents had taken her to Kuliyapitiya Teaching Hospital, where she was pronounced dead on admission.
A postmortem examination could not ascertain the cause of death.
Body samples have been sent to the Medical Research Institute for examination.
